“I had a BA silicone 450 R went to 500 /500 went to 550 L under, on March 21, I want a redo to go bigger by 100 cc’s , how long should I wait to get a redo, I was thinking maybe July or wait til Nov of this year.”

This is probably one of the most common complaints after Breast Augmentation not related to a complication. Many patients feel that their Breast Implants are too small right after their surgery. Personally, I have never felt this way (even after two different breast augmentations with 510 cc saline breast implants) because I had gone rather large and admittedly regretted choosing such a large size (both times!). Granted, I liked it at first. When you’re just lying around healing, having noticeable results is like Christmas! But when you get back to your daily life, finding bras that fit (32F!) and easing back into your activities can be a little “different” with that much mass on your chest. So be careful what you wish for. And remember, the larger you go the more stress is applied to your tissues and the faster you will sag without sufficient support (although we all sag eventually).

Now that you’re heard my thoughts from personal experience, I thought I’d share the comments of 4 of our Expert Breast Augmentation Surgeons…

Thank you for your inquiry. I would recommend that you wait at least 2
months. This will allow most of the swelling to go down and tissues to
have healed enough. Also allowing the overlying tissues to relax may
change the appearance enough that you may not want to go bigger. I have
many patients who think their breasts are too small immediately after
surgery and as things settle down they end up being very happy. Especially
with the implants behind the muscle.
Sincerely,

Dear K,

I received an email from Marianne and she requested I answer this
question for you. It is a pleasure as I know Marianne for years and know
how she cares for patients and visitors to her site.

For your revision surgery, if you have existing good results with the
large size you already have in, then going 100cc larger is small by
comparison to the size implant you have and should cause no problem and
add little or no risk. Timing for revision surgery is a judgment call
but can be as soon as your tissues are soft (to allow safe surgery) and
you are not swollen (to be a good judge of size). Also make sure you
have no issues with your current result except for size. I have done
this surgery as early as six weeks in some patients but usually is best
at between 3 and 6 months depending on the factors just mentioned. Be
careful, on occasions to go even slightly larger may mean that slight
lowering of the IM fold is needed or scoring the capsule to expand the
space to still keep the NAC centered.

Dr. James Romano – San Francisco, California Board Certified Plastic Surgeon

______________________

A common mistake some women (and surgeons) make is to do too many surgeries too fast. There are a number of questions that need to be asked. Did you change what you wanted? Did you not get what you expected? Why do you want to be bigger? Can you be happy where you are? Do they look good now? This needs a thorough and honest discussion. The decisions that go into what procedure to do are just as important as knowing how to do it. Patience here is a virtue. I would wait 9-12 months before embarking on another breast implant procedure (as long as things have healed up well, etc). Good luck!

______________________

Dear K,

If you just want to increase the size of your implants by 100cc, you could do it almost at any time. I would suggest that you think if that would accomplish what you want for your cup size would increase by about ¾ size. Also, your breast size will still continue to diminish over the next several months. 120-150cc is about one cup size. If 100cc is all you wish to add, then if the implants are saline, I can even do this under a local anesthetic in our office operating room. I can usually increase the implants size by up to 400cc but I advise waiting at least six months for that. Please e-mail us back if you have further questions.

Follow-up:

The incision under the fold makes it more difficult to fill under local anesthesia for I need to turn the implant and get to the valve port. It can definitely be done but probably with IV sedation. I usually use an incision around the areola for I feel it heals better, gives me better access to do the surgery and the loss of sensation is the same as with any other incision. The one under the breast does heal well in most patients but if it doesn’t (in about 10% of patients), then it can be a problem, especially when the swimming suit or bra rides up it becomes visible. Thanks! Dr. Ed Domanskis

Although surgery preparation in the weeks before your tummy tuck surgery may seem overwhelming, if you plan your steps and create lists for supplies and tasks, this will significantly lower your stress levels. Doing so helps you focus on relaxing and healing, not worrying, which can only enhance your recovery. Plus, you are sure not to forget anything! The last thing you need on your surgery day is having to stop at the pharmacy and wait in the car while your caretaker rushes around searching for a forgotten item. These tips, along with your own surgeon’s instructions, should definitely help you avoid that inconvenience.

Having experience with plastic surgery is definitely a benefit in the planning process as I wasn’t always so thorough. Thankfully my surgeons and their staff helped guide me along in the process when I had my first few procedures. It certainly is a lot easier now. Truth be known, I have a storage container with plastic surgery recovery items alongside my various holiday decoration storage boxes. Also several Restylane and Juvederm coldpacks ready to go in my freezer.

Although the little details won’t make or break your experience, they can assist in providing you with better comfort and less overall stress.

Outlining Your Tasks & Timeline
Although your surgeon will provide you with many of these instructions, these lists were compiled with the help of real patients and reviewed by their (and my) surgeons for accuracy. They are provided for you to cross-reference or add to your own lists, and not intended to replace the advice of your surgeon. Please speak with your surgeon before changing or adding anything to your recovery protocol.

Medications & Supplements To Avoid Before Surgery: There are many medications which may interfere with your body’s ability to clot normally. Others may interfere with the anesthetics during your surgery. Also, just because it’s an over the counter pain reliever (OTC), or that a vitamin or homeopathic may seem “natural” and not a prescription drug, they can still interfere with platelet formation, whether not enough (blood thinning) or too much (thickening). Be sure to ALWAYS divulge every possible medication, supplement or homeopathic that you consume even if it has been in the last few weeks. This list is a partial list and does not imply that you should quit taking any prescription medications, it is just a sample list of the medications which may cause problems so be sure to let your surgeon and anesthesiologist know about it.

Pre-operative Shopping List: Making a list of what you will need for your surgery will help cut the stress significantly. Items you will purchase will include your prescriptions, your compression garments, bags of frozen peas, various wound care supplies, and a host of other items which will certainly make your recovery go more smoothly.

Preparing Your Recovery Area: Preparing your recovery area in advance will ascertain that you will have all the necessary items within reach. An area for your medications, a cellphone charger, laptop and laptop charger, Kleenex, bottles of water, a digital thermometer, a medication time chart, a plastic pail in the event that you become nauseated, etc.–details do, indeed, matter when you aren’t able to move around as well as you normally can. This list is rather extensive and is only meant as a suggestion, although some items are obviously necessary. Preparing this area in advance will definitely allow you to have time for yourself to prepare emotionally and relax, as well as will let you know which supplies you may be lacking, if anything.

Emotional Preparation & Reminder Checklist: Undergoing surgery, requires mental preparation and patience. Preparing yourself in advance will help you keep your emotions in check after your procedure, when you will need it most. A good support system is such a huge plus. If you don’t have good support at home, please visit our Tummy Tuck Discussion Forum. We’ll make you feel right at home! We do NOT have the mentality that you “deserve” to be in pain because you “chose” to have a cosmetic procedure. That is nonsense, you’ve been taking care of the family unit and everyone else for how long now?? You deserve to be queen for a while and you deserve to feel good about your body again.

Surgery Day Instructions Example: This list helps you know what to expect and how to prepare your vehicle for the ride back home (if you are not staying in a recovery center). This list provides basic instructions as well as tips from experience.

Helpful Tips and Suggestions For After Your Surgery: This list is quite extensive, although you should have your own surgeon check over it before you follow anything outside his or her specific recovery protocol. Many of the tips are from the collective experience of other tummy tuck patients.

What To Do In The Event of An Emergency During Recovery: Your surgeon should provide you with a list of symptoms which may indicate an emergency, this list is an example and was given to me by my own surgeon. You should definitely run it by your own surgeon to ascertain that he agrees. Serious complications are rare, but it is better to be prepared.

Preparing For Your Post-operative Appointment: Your follow-up appointments are very important. They provide your surgeon with feedback on your physical recovery and your emotional state and satisfaction. Your first post-operative appointment will usually be for drain removal which may or may not coincide with your suture removal. Your second will be a follow up on your recovery and to gauge how your results are coming along. Depending upon how invasive your surgery was (skin only versus muscle repair, or considerable tissue removal) your surgeon may require you to come in more or less frequently. He or she will let you know. Regardless, it is good to have your list of questions, complaints or concerns outlined with full documentation of anything out of the ordinary so that you will remember to bring it up at your appointment.

Even after reading this article, you still might feel overwhelmed. But I know you are also excited about your procedure! With the help of these printer-friendly guidelines and tip sheets, which also includes a section on how to prepare for your consultation or what to ask your surgeon if you are still in the early stage of your tummy tuck journey, it will be a lot easier.

To view the checklists and download the PDF files, please see our tummy tuck preparation page: http://tummytuckinfocentral.com/preparation.htm Or visit our website http://www.TummyTuckInfoCentral.com to start the Tour To Tummy Tuck Surgery, for info on the beginning to the end of your abdominoplasty procedure. Our website includes before and after photos of tummy tuck patients, a board certified surgeon locator, patient journals, a tummy tuck discussion forum and more! If you have any questions, comments, or suggestions, please feel free to contact me.

Before: Damage After Hairlocs Removal. After: Fusion: Hate It And It Looks Awful, Too.

I once read a comment by some random celebrity (I can’t remember who, now) “The fastest way to having short hair, sans cutting it off, is to get hair extensions.” Truer words were never spoken. I have documented my experience with hair extensions over a 5 year period with 9 different systems so that you can better avoid the problems that I had. I no longer have extensions but I have had enough experience with many methods to have an informed opinion about it. I am not trying to dissuade anyone from getting them, I just want you to know what you are getting into with each method and the reality of its effects on your natural hair. Hair extensions are not for everyone.

I sustained considerable hair damage, a huge dent in my pocket book but walked away with a ton of personal experience.

Something you may not know about me, but I am a bit of a foodie. Gourmet cooking is a hobby and I prepare gourmet meals every day. Whether it is Porkchops with caramelized onions and sweet potato gravy or Chicken Involtini stuffed with feta, golden raisins, proscuitto and collard greens. Oh yeah and Pine Nuts. As is customary, I snacked on a few while cooking. Yum. Wait. Not so yum. These just aren’t as good as I remembered. But, whatever, this is what we’re having for dinner. And it was fabulous when we ate it. However!

Almost two days later I started getting the most bitter, metallic taste that would just not go away. My nightly glass of cabernet was awful. Brushing my teeth didn’t get rid of it. Bananas Foster French Toast the next morning? Forget it. It was disgusting. Vitamin water was able to help, but only for a few minutes. What was going on??

Then I started borderline panicking, Can I somehow taste the infinitesimal amounts of Cesium 137 that has been found in our drinking water from the Fukushima crisis? No, of course not. Cesium 137, or Iodine 131 for that matter, are both tasteless and odorless no matter the amount. Why can’t I taste anything but metal!!! I don’t have metal fillings. Perhaps I have too much iron in my system? No, that’s impossible. OMG, what is it? Do I have salivary gland or tongue cancer? Am I dying????

It never even occurred to me that it was related to the nuts because I used to eat them often, although it has been about a year or so though. The package sat on the counter and I occasionally snacked on them for the last week. And still this taste remained. NOTHING helped, no amount of sweet, spicy, or any other taste in between could get the taste out of my mouth for more than a second or two. I have been complaining to my boyfriend about it since the onset and he is not experiencing any complaints. He only had the cooked nuts within the Chicken Involtini, I had been eating them raw and I have eaten a lot more than he has.

I didn’t Google anything about it until today (because I can simply no longer handle it!) “bitter taste in mouth after eating + symptoms”. Then I found a news report (‘Pine Mouth’: How Pine Nuts Can Ruin Tastebuds for Weeks) about Pine Mouth and pine nuts from China. I thought when they mentioned Chinese pine nuts that I was exempt because I remembered seeing “Pignolias” (In festive red, white and green no less) on the front. Of course, I mistakenly assumed that they were from Mexico as I often get Mexico-grown dried spices. I mean, how often do we look for the origin or specificity of less “important” ingredients. If a recipe calls for Japanese eggplant, of course I would get Japanese eggplant . If the martini called for an English cucumber, no other would do. Obviously growers in the US produce these eggplant varieties, but some produce items are actually imported, such as the mango. India produces half of the world’s mango supply. But I digress. My point is that now I have to check the origin of a nut to determine its quality and if it is even a foodstuff to begin with? The rumor is that China may be exporting non-edible pine nuts. Well, lo and behold I looked on the package and they are from China, distributed by Stater Bros.

So I decided to write a blog on it and then I came across this blog article: Pine Nuts Cause Bad Taste in Mouth: Pine Mouth Syndrome and some more news reports and personal accounts of the very same thing. I had tried calling Stater Bros but Customer Service is closed on the weekends. What is interesting is the FDA started getting an alarming number of complaints in 2009 to present. I don’t recall this EVER happening to me until last week. Gourmet cooking is my hobby and now I can’t taste anything at all but metal. I made one my favorite Italian dishes and I was mortified. The reports have said that people have not experienced relief until anywhere from 1 week to 4 weeks after ingestion! I will be complaining to the FDA and calling Stater Bros Distribution first thing Monday morning. I know it isn’t Stater Bros’ fault, but SOMEONE isn’t doing their job somewhere along the line, whether that be in customs or USDA or elsewhere.

Why Report? http://www.fda.gov/Safety/ReportaProblem/default.htm

Number to call and complain to the FDA: http://www.fda.gov/Safety/ReportaProblem/ConsumerComplaintCoordinators/default.htm

I have read more from all over and it appears that a lot of the couples which shared the same meal, the person who was cooking (ergo, SNACKING during cooking. I mean who doesn’t snack on Pine nuts when they’re cooking?) was the one who suffered the most, if not the only person of the two. It is the same with us. He only had the ones that were toasted AND cooked within the chicken in the oven. I was a glutton.

I just can’t wait for this to be over. Honestly, no one really knows why, how or from where, but for now, just be sure you know where the nuts came from. Thus far, it is suspected to be from China. Just do your own research and also visit the above links. The blog article, Pine Nuts Cause Bad Taste in Mouth: Pine Mouth Syndrome, is excellent so be sure to visit. And in the meantime, hold the nuts!

Updated:

I thought this was Interesting, “Non-edible pine nuts of the species pinus armandii and pinus massoniana originating in China have been imported onto the local market.” (Malta)

http://www.maltatoday.com.mt/news/national/non-edible-pine-nuts-from-china-on-the-local-market

UPDATE: April 4, 2011

I contacted the FDA Southern California Complaint Coordinator (949) 608-3530. I will be sending them the product and they are going to be getting back to me.

I also spoke with the store manager for my local store and told her about it. She said she’d pull them from the shelves but I doubt she can until corporate contacts her and gives her permission.

I contacted Stater Bros. Markets Customer Service: (888) 992-9977
at 5:21 PM today to make report and spoke with the operator (Anna) and was told that the buyer, Jim Musgrave, would contact me. I gave her some information but I am sure that he will have to do his own investigations. I assure her it wasn’t dangerous, just that it caused a bad taste in the mouth for a while.

I get the feeling people think I am nuts but the facts are there, it’s happening everywhere and I am thrilled to announce my sense of taste has almost returned entirely! I was half tempted to eat them again just to be double sure and to document the amount eaten to cause the disruption, the onset time, the symptoms and the time that it took to return to normal. But I thought to myself, nahh. I’m just not that curious.

UPDATE: April 5, 2011

I had only one weird taste after I initially ate my first bite of spaghetti and meatballs last night. One of my favorites. I was disappointed but after another bite it seemed to be fine.

Frank Lee, the Southern California FDA Complaint Coordinator called me today and took my report. I am again awating another call back. Thank God this isn’t dangerous. Which reminds me, after witnessing a vehicle flip on the highway that had a toddler and a mosth as passengers other day I tried to call 911 and had a busy signal several times. Another lady was able to get through. Madness.

UPDATE: April 8, 2011. No call back from the FDA. Seriously.

Don’t you love the ads, “Wake up looking as though your make-up was freshly applied” or “never put make up on again”? Whatever. Obviously they weren’t referring to my face in the mornings. I assure you, even with permanent makeup, I don’t look like Jane Diva when I crack my eyes open for the first time each day.

The one thing that really grabs your attention and convinces you to go for it is the fine collection or just-been-tattooed before and afters. Especially on the clients with pale blue or gray eyes. Put dark eyeliner on any light eye and you’re gonna look like a super model. The tattooed eyeliner looks especially dark and perfectly applied right afterwards, plus comparing it to an unmade up eye is simply night and day.

Granted tattooed brows are often dark and clown-like right afterwards and I don’t care what they say, if you don’t have enough brow hair to camouflage, it won’t look natural. I have had permanent eyebrows for 5 years, I know.

And the lips? The lips always look super bright and are as shiny as a thirteen year old slathered in dollar store lip-gloss. Wait. I sometimes like inexpensive lipgloss. Scratch that. Hwo about a 5 year old that has gotten into her Mom’s Avon samples?? Whatever the case, I never decided to get my lips done, and I am so glad that I didn’t.

Okay, so why did I do it if I am so bitter? Because I had sparse eyebrows and got tired of taking 30 minutes to apply symmetrical penciled-in-brows every day. And while I was there, I figured I’d get lash enhancement—not exactly eyeliner but little dots of color around my lashes to make them stand out more. I actually originally wanted permanent lip color before anything else to make my vermilion border more prominent. But, after seeing and hearing about the tons of faded, messed-up hack jobs I just wasn’t willing to take that chance. Lip liner pencil isn’t all that difficult to apply. And honestly I don’t even use lip liner anymore.

The one thing many permanent make-up artists don’t take into account is how the pigment will appear through the color of your skin. If you have a yellow skin tone, well that bluish-black ink will look green. If you are fair as Snow White, the color is more likely to stay true. But your skin color does determine the end result. Remember the pigment is implanted into your skin so as soon as it heals, you will have a layer of skin tone between it and someone’s eyesight. Remember the color wheel, my friends.

Another issue I have with permanent make up is it’s permanent! My tattoo artist gave me some hogwash about brows being like two sisters, not twins. That’s just an excuse for people who can’t tattoo symmetrically. I have reviewed a ton of photos, including my own, and it is my opinion that symmetrical brows look better than asymmetrical ones. You cannot sway me with your sisters-logic!!! I don’t want to look sexy on one side and frowning on the other. It’s just not right. Make SURE you use stencils or some form of system that you can get what you want and always go lighter than you think. Simply drawing your own on and having them wipe a bit of your brow pencil off and tattooing it back in is not sufficient. Which is exactly why I am not happy with my own.

Also, as the pigment fades, you look uneven. As you may already know permanent make up is not all the way permanent. It’s just sorta-kinda permanent. Permanent enough to where doesn’t go away, but semi-permanent enough that it gets splotchy. Joy. The inner areas are what have faded the most and will continue to slowly do so. Why can’t my regular tattoos do this? Well, at least the ones I am trying to get rid of.

Now before I brace for a barrage of negative comments, if you get a good permanent make up tattoo artist and you keep up with the maintenance touch-ups, you will be happier than I am. It is possible to look pretty good with permanent make up. But it’s usually a one shot deal. After the first mistake corrections are difficult. I am just thankful mine have faded so much. The outer area is what I don’t like the most because it is truly a faint tattoo line and thin. I have had several people ask, “Are your eyebrows tattooed on?” I say, “Yes.” Then they ask, “Why did you do that?” Then I have to go through the anguish of explaining the whole story about why I did it, then how I regret it, blah blah blah.

The pain factor: It really doesn’t hurt too badly if you use a topical anesthetic like LMX. The lashline tickles like crazy however. You can’t feel the needle because of the anesthetic eyedrops but I tell you what, you can feel the vibrations. The eyebrows don’t hurt at all if you use an anesthetic. Just be careful with topical anesthetics on the mucosa of the lips, I have a friend who had a serious reaction. One more thing, be sure the technician does not add the anesthetic to the ink itself. This can be dangerous.

Cosmetic facial tattooing is an art, it truly is. So please be sure to do your research. Another tip I have is to bring a digital camera. That way you can truly see how the brow shape or new lip shape will look like once it is tattooed in. When you look in a mirror, your eyes can adjust and not see a true perspective. Take a digital photo to truly see how it looks, use these free eyebrow stencils, anything. Don’t be stuck with something you don’t want.

I first sought treatment for under eye hollows with Sculptra® in November of 2004 and again for my second treatment and also to nasolabials in February of 2005. It lasted for about 18 months and since I had developed a nodule from the Sculptra®, I decided to try a product which I felt had less possible complications, Restylane.

So August 30, 2006 I had my first Restylane® under eye injections with Jonathan Hoenig, M.D. in Beverly Hills, California. Incidentally the above photo was from a treatment in September of 2009. I get Restylane injected under my eyes about once a year.

I have also been treated with 5FU injections (starting 8/2006) to correct the scar tissue between my left cheek and under eye from the fat grafting procedure I had in 2004. Granted, I had the demarcation prior to any facial procedures, but the fat grafting seemed to accentuate it.

The Scultpra page has been updated with more information and personal treatment photos. I first had Sculptra in late 2004 and again in early 2005 as part of a clinical study and used it for cosmetic applications for the cheeks, undereyes and nasolabials.

What Is Sculptra?
Sculptra is poly-l-lactic acid, a cross-linked lactic acid product which is injected into the dermis to trigger collagen production. Sculptra is a semi-permanent product designed to create volume and thicken the skin via natural collagen formation and lasts about 18 months.

Sculptra Aesthetic® is FDA approved for cosmetic applications for the nasolabial folds and for other deep dermal wrinkles. It is also used for cheek volume and along the jaw for augmentation. It should not be used in the lips, and in thin-skinned areas like under the eyes (although I had gotten it there during clinicals).

Sculptra was originally approved only for patients with lipoatrophy, which is the loss of facial fat/volume from medications and disorders such as those for HIV. Lipoatrophy is also known as facial wasting and can cause a gaunt, unhealthy look. However, it was used off-label cosmetically. Now that Sculptra Aesthetic® is FDA approved for cosmetic applications, patients can readily receive injections from their plastic surgeon or dermatologist.

I love full, high cheeks. I naturally have prominent cheeks, but I am always in the mood for more. I had 1.3 cc of Radiesse to both cheeks on January 2010 by Dr. Jonathan Hoeing in Beverly Hills, California. I loved the result and there was no palpable lumping at all. I had only slight swelling, swelling that I appreciated and was sad to see dissipate. As I said, really love big cheeks and full lips, so this area is one of those where my doctor knows best. I get a case of cheek or lip greed every now and then, and my doctor keeps me in check.

Unfortunately, my first Radiesse experience wasn’t this successful. I first had Radiesse® injected in 2004, although back then it was known as Radiance FN® and it was into my lips. I had heard so much about the product and felt at the time that it would be a good option. I was told that it should last about 2 years and that in the long run, I would save money that I would normally spend on temporary fillers every 4 months. The distributor had even comped the product (it was for a documentary on Discovery) so I was excited all over the board. Well, unfortunately I had blanching and palpable lumping (both visual and with manual manipulation) which necessitated removal so needless to say I wasn’t too sold on the product at first. These were the early days so some of us had to take the fall for the learning curve. Radiesse is no longer recommended for lips. To read more on this, see: Radiesse Removal.

But I think Radiesse is a great product for dermal facial augmentation and volume restoration!

Learn more about how Radiesse can help you restore lost volume or create augmentation where you never had it before…
